Physical Therapist - Full Time

Minimum Requirements:

Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited school approved by the APTA.Current Wisconsin Physical Therapist (PT) license.Experience working with adult and geriatric populations in skilled nursing or long-term care settings is preferred.Experience with documentation and compliance; experience with Net Health is a plus.BLS certification from the American Heart Association or American Red Cross may be required.Strong communication, teamwork, and professional conduct.

Your Responsibilities

Provide compassionate and effective physical therapy services to adult and geriatric residents in a skilled nursing facility.Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence.Support residents in achieving personal goals and rehabilitation milestones.Accurately document care and maintain compliance standards.Collaborate with therapy and nursing teams to ensure patient-centered outcomes.Educate residents and families on safe techniques and strategies to maximize independence.Participate in team meetings and contribute to care planning.

What You Bring

Current Wisconsin PT license.Strong clinical skills and a focus on working with older adults.Excellent communication and teamwork abilities.Flexibility, professionalism, and a positive attitude.
