Work Environment

This role is performed onsite, primarily in patients' homes across the Brookhaven area. The work setting is characterized by individualized one-on-one care, allowing for deeper relationships and more tailored interventions. Key Responsibilities

• Conduct thorough assessments to identify patient goals, functional limitations, and specific requirements.
• Manage an average caseload of one patient per hour, ensuring that each session is focused and productive.
• Design and implement individualized treatment plans utilizing evidence-based techniques that improve mobility and function.
• Utilize manual therapy techniques and exercise protocols to enhance patient's range of motion and alleviate pain.
• Document all patient interactions, treatments, and progress clearly and accurately in electronic medical records (EMR), specifically utilizing WebPT software.
• Collaborate with healthcare providers, including physicians and specialists, to ensure comprehensive patient care.
• Motivate patients and encourage adherence to treatment plans while providing education about healthy lifestyle choices.
• Monitor treatment outcomes and adapt care strategies based on patient progress and feedback.
• Work towards reducing dependence on medications through effective therapeutic interventions.

Patient Population / Client Focus

Your primary focus will be on patients requiring orthopedic rehabilitation. This population may include post-operative recovery patients, individuals with chronic pain conditions, or those rehabilitating from sports-related injuries. By providing targeted interventions in the comfort of their own homes, you will facilitate a more supportive environment for recovery, which can significantly enhance outcomes.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• A valid state license to practice as a Physical Therapist, with an exemplary professional record, free from board or disciplinary action.
• Proficiency in manual therapy and exercise prescription techniques.
• Excellent verbal and written communication skills to effectively interact with patients and multidisciplinary teams.
• A strong commitment to providing high-quality, evidence-based care that emphasizes patient engagement and education.

Preferred Experience

• A minimum of 2 years of experience as a Physical Therapist, preferably within an orthopedic outpatient or home care setting.
• Familiarity with EMR systems, specifically WebPT, and strong documentation habits.
• Experience in developing and managing individualized exercise and wellness programs tailored to diverse patient needs.

Compensation and Benefits
