Compassionate Care Hospice San Diego is dedicated to providing exceptional hospice and palliative care services to individuals and families navigating end-of-life journeys. Located in the San Diego Metropolitan Area, we focus on delivering holistic, patient-centered care that prioritizes comfort, dignity, and emotional support. Our team of professionals is committed to tailoring care plans that meet each patient’s unique needs while offering guidance and compassion to their loved ones.
Role Description
This is a full-time, on-site role in North Coumty San Diego for a Hospice Liaison. The Hospice Liaison will work with healthcare providers to establish and maintain relationships, educate the community about hospice services, and help identify patients who may benefit from hospice care. Day-to-day tasks include representing the organization in the community, fostering relationships with referral sources, providing information about hospice and palliative care services, and collaborating with the care team to ensure the best possible patient outcomes.
Compensation
80-120 k
Base with outstanding bonus structure, 401k , health insurance and other benefits.
Qualifi c ations
- Expertise in Hospice Care, End-of-Life Care, and Palliative Care
- Sales experience and ability to build strong partnerships with referral sources
- Knowledge and experience in Home Care and the broader healthcare system
- Outstanding communication and interpersonal skills
- Ability to work effectively in a team-centric, compassionate environment
- Proven organizational and time-management skills
- Experience in community outreach and education is a plus
- Current licensure as a healthcare professional is preferred but not required
- minimum 2 year experience in Hospice Marketing/sale
