Nurse Practitioner for In-home Visits

At Signify Health, part of CVS Health®, our mission is to build trusted relationships to make people healthier. Position Board‑certified Nurse Practitioner Contractor. Conduct in‑home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. Responsibilities Conduct in‑home visits that focus on a person’s overall health and well‑being, identifying potential barriers to care. Provide a medication and medical history review and a physical evaluation performed in the member’s home. Use pre‑populated, patient‑specific forms for each evaluation. Do not prescribe medicine, order lab tests, or alter the member’s current treatment regimen. Compensation Earn up to $2,500 to $4,000 per week (average 6–9 visits daily, base rate of $80 per completed assessment). Increase per‑visit rate up to 135% with our Market Multiplier for Medicaid license or willingness to enroll. Additional compensation via pay‑per‑test model for qualifying visits. Availability incentives based on weekly availability. Schedule & Work Conditions Shifts available Monday–Sunday (no nights/weekends required). Short‑term or long‑term basis. Pre‑scheduled in‑home visits with health plan members. Airfare, hotel, and rental car available (depending on eligibility). Benefits & Requirements Independent Contractor (1099) status.
