Health Home Care Manager

Human Development Services of Westchester (HDSW) is a Westchester County community-based not-for-profit, has been involved in the evolution of community-based direct-care services for vulnerable populations in New York State since 1968. In todays ever-changing healthcare marketplace, HDSW offers person-centered, trauma-informed, culturally competent services to over 3000 people each year, through a unique blend of innovative inter-related programs. Position Overview:
To assist clients living in the community to link to necessary medical, behavioral health, housing, social, employment, educational, and other services essential to meeting basic life needs.
Sign-on Bonus: $3000 paid out over the first year of satisfactory employment.

Qualifications:
- BA/BS in a human services field with a minimum of one-years experience in a behavioral health setting is required OR AA/AS in a human service field with a minimum of two-years experienc e in behavioal health setting.
- Must have understanding/knowledge of the behavioral characteristics and psychosocial needs of the target population.
- Strong written and verbal communication skills are necessary to maintain documentation and work effectively with clients, providers, and the healthcare community.
- Skills & Knowledge Behavioral health
- Bi-lingual English and Spanish speaking are a plus.
- Must have own car, a valid drivers license, and a driving record that is satisfactory to our insurance carrier.
- Criminal History Background Check and fingerprinting are required.
