Job Duties and Responsibilities Performs comprehensive patient assessment , including health histories and physical examinations, to identify patient needs. Formulates accurate diagnosis of acute and chronic health conditions based on clinical findings and diagnostic tests. Develops and implements individualized treatment planning , integrating evidence-based practices and patient preferences. Applies principles of pharmacology to manage medication regimens safely and effectively. Exercises authority in prescribing medication and other therapeutic interventions, adhering to regulatory guidelines. Maintains meticulous and timely patient records using Electronic Health Records (EHR) systems. Provides thorough patient education to individuals and families, promoting health literacy and self-management. Demonstrates exceptional communication skills when interacting with patients, families, and healthcare colleagues. Engages in effective interprofessional collaboration with physicians, nurses, and other specialists to optimize patient outcomes. Manages and monitors patients with chronic disease management , focusing on prevention, early detection, and long‑term health. Required Qualifications Education Completes a Master's or Doctoral degree from an accredited Nurse Practitioner program. Holds current Nurse Practitioner certification from a recognized national board. Possesses an active, unrestricted state license as a Registered Nurse and Advanced Practice Registered Nurse. Experience Demonstrates a minimum of 2 years of clinical experience as a Nurse Practitioner in a relevant specialty. Exhibits experience in an outpatient or inpatient setting, managing diverse patient populations. Knowledge/Skills Applies advanced knowledge in patient assessment techniques and methodologies. Possesses expert ability in diagnosis and differential diagnosis of various health conditions. Develops and executes effective treatment planning strategies. Understands and applies principles of pharmacology in clinical practice. Proficiently exercises judgment in prescribing medication and other treatments. Navigates and utilizes Electronic Health Records (EHR) systems efficiently. Delivers clear and comprehensive patient education . Excels in communication skills , both verbal and written. Engages effectively in interprofessional collaboration . Manages chronic disease management with a patient‑centered approach. Demonstrates strong critical thinking and problem‑solving abilities. Exhibits empathy and a patient‑first approach to care. Preferred Qualifications Possesses additional certifications in a specialty area (e.g., Family, Adult‑Gerontology, Acute Care). Shows experience with quality improvement initiatives or research. Physical Requirements/Work Conditions Works in a fast‑paced clinical environment, requiring standing, walking, and sitting for extended periods. Lifts and moves patients or equipment up to 25 pounds occasionally. Performs fine motor skills for examinations, procedures, and computer use. Maintains visual and auditory acuity for patient assessment and communication. Adapts to varying work schedules, including evenings, weekends, or on‑call duties as needed. Manages exposure to communicable diseases, blood‑borne pathogens, and other medical hazards with appropriate precautions.
