Bilingual Health Navigator for HIV Care & Prevention

San Francisco AIDS Foundation seeks a Health Navigator to facilitate access to HIV prevention, care, and supportive services at Strut in Castro. You’ll welcome clients, assess needs, provide referrals, and help navigate benefits and entitlements in a trauma‑informed, culturally competent environment. You will collaborate with nursing, clinical, housing, and benefits teams to coordinate follow-up and linkage, ensuring clients remain engaged in services.
