Job Title: Care Partner Location: The Esther at Riverbend Job Type: Fulltime/Part-time & Day/Swing (Evening)/ Noc (Night) Reports To: Resident Care Coordinator/Health and Wellness Director --- Job Overview: We are seeking a compassionate and dedicated Care Partner to join our team. The Care Partner plays a pivotal role in providing high-quality care and support to individuals in need of assistance due to physical, emotional, or cognitive challenges. This position is ideal for someone who is passionate about enriching the lives of others and committed to delivering personalized care in a respectful and supportive environment. --- Key Responsibilities: - Personal Care Assistance: Provide support with daily living activities, including bathing, grooming, dressing, and toileting, ensuring the dignity and comfort of individuals in your care. - Emotional Support: Establish and maintain positive, trusting relationships with clients, offering companionship and emotional support to enhance their quality of life. - Medication Management: Assist clients in managing their medications as prescribed, ensuring accurate understanding and adherence to medication schedules. - Mobility Assistance: Help clients with mobility needs, including transferring, walking, and using assistive devices, to promote independence and safety. - Household Support: Perform light housekeeping duties, including cleaning, laundry, meal preparation, and grocery shopping, to maintain a safe and comfortable living environment. - Communication: Serve as a liaison between clients, families, and healthcare providers, effectively communicating any changes in health or behavior to the appropriate parties. - Documentation: Maintain accurate and timely records of care provided, client progress, and any concerns or observations, ensuring compliance with organizational policies and regulations. - Team Collaboration: Work collaboratively with other care team members, attending training and meetings as required to ensure continuous improvement in care delivery. --- Qualifications: - Education: High school diploma or equivalent preferred. Completion of a certified nursing assistant (CNA) program or relevant training is a plus. - Experience: Previous experience in a caregiving role, such as home health aide, nursing assistant, or similar position, is preferred. - Skills: - Strong interpersonal and communication skills to engage with clients and families effectively. - Empathy and understanding of the needs of individuals with varying levels of care requirements. - Ability to handle potentially stressful situations calmly and professionally. - Certifications: CPR and First Aid certification preferred.
