Substance Use Counselor
Personal Touch Home Health Care Services Inc.
We are seeking a Substance Use Disorder Counselor to provide comprehensive treatment and support for individuals overcoming addiction. Responsibilities include delivering individual and group counseling , conducting assessments, screenings, and treatment planning , coordinating case management and community services , maintaining accurate clinical documentation, and collaborating with multidisciplinary teams and community partners. Qualifications require a valid Wisconsin SAC-IT, SAC, or CSAC credential , an associate's degree in a related field (bachelor's preferred), and knowledge of addiction, recovery, and case management principles. Ideal candidates have experience with justice-involved populations , a valid driver's license , and reliable transportation .
