Key Responsibilities\n

The Licensed Professional Counselor will take on numerous responsibilities critical to client outcomes, including:

• Providing evidence-based interventions for patients requiring mental health services, offering brief and focused sessions that promote emotional and psychological growth.

• Maintaining accurate and up-to-date clinical records, documenting each patient's progress and diagnostic information in compliance with Agency and Federally Qualified Health Center (FQHC) guidelines.

• Coordinating care with other treatment providers, ensuring that patients receive comprehensive support tailored to their unique situations.

• Monitoring multiple patients’ progress, adjusting treatment approaches as necessary, and developing relapse prevention plans for at-risk individuals.

• Offering education and self-management skills to patients, empowering them to take an active role in their ongoing recovery process.

• Engaging in collaborative discussions with primary care teams to enhance the treatment plan based on patient feedback and clinical observations.

• Participating in team meetings to discuss behavioral health services and potential areas for program improvements.

• Serving as a resource for clinic management and staff concerning behavioral health insights and trends.

• Adhering to the Code of Ethical Conduct for Clinical Professionals while fulfilling other duties as assigned.

Qualifications\n

• A minimum of a Master’s Degree in Clinical Counseling or a related field.

• Current and valid LPC (Licensed Professional Counselor) license.

Compensation and Benefits\n
