Job Details:

• Join a well-established adult primary care practice in a collaborative, team-oriented environment
• Open to Nurse Practitioners (FNP) and Physician Assistants (PA)
• 100% outpatient primary care setting focused on adult medicine
• Full-time, Monday-Friday schedule with no call required
• Ask about a 4-day work week option!
• Perform routine physical examinations and formulate medical, educational, and psychosocial plans to treat patients
• Develop plans for further evaluation of health needs, including referrals to other health team members or specialists
• Provide appropriate treatment for problems within scope of practice and refer appropriately when needed
• Utilize EPIC and state-of-the-art clinical and informatics technology to promote improved patient outcomes
• Access to dedicated support through an Office of Advanced Practice led by system-level leadership
• Established mentorship and orientation programs specially designed for newly hired providers and providers new to practice
• Ongoing professional development through a robust on-demand learning platform including CME, special courses, and learning series
• Requires unrestricted State of Connecticut medical license (or eligible)
• Requires APRN (Family Nurse Practitioner) or PA training with 1+ years of experience in primary care or a relevant outpatient setting
