Key Responsibilities\n

• Conduct comprehensive psychosocial assessments of patients to evaluate their emotional, social, and environmental needs through interviews, observations, and medical record reviews.

• Provide individual and family counseling, addressing psychosocial issues, emotional challenges, and adjustment to life changes following rehabilitation.

• Collaborate with the rehabilitation team to create tailored treatment plans that address specific psychosocial needs while aligning with clinical goals.

• Assist patients in identifying and accessing appropriate community resources, including support groups, vocational rehabilitation, or housing assistance.

• Educate patients and families about available community resources and guide them through post-rehabilitation services.

• Actively participate in discharge planning, ensuring smooth transitions for patients returning home or to alternative care settings.

Patient Population / Client Focus\n

Our client base consists of individuals recovering from various medical conditions, including those with substance use disorders, severe mental health challenges, or those undergoing detoxification. You will work with patients from diverse backgrounds, each requiring personalized care that addresses their specific emotional and psychological needs.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Active state licensure as a Licensed Clinical Social Worker (LCSW) in New Jersey.

Compensation and Benefits\n
