DIRECT HIRE Physical Therapist PT Home Health - Port Charlotte, North Port 40 hours per week, working days, some weekends roughly. Will discuss further in interview. Pay is on a point system, overall hours will vary based on cases and types of visits. Salary is based on location and skill level.

• While annual income related discussions will be had during the interview process with a candidate, based on their overall experience, these positions will likely range from $50-$55/hour, or produce an annual salary of $105,000-$115,000.
• Candidates do need to remain flexible with the territory that they will be providing care within; however, Empath will never require an employee to travel an unrealistic distance. For example, someone assisting within the Sarasota area will not be asked to travel as far south as Naples.
• An average Physical Therapy visit is approximately 90 minutes in length; therefore, 4-5 visits per day is a typical expectation.
• It is important that candidates do hold prior home care experience.
• From a sign-on bonus perspective, there are certain factors that will determine whether this is a feasible incentive to include within an offer being issued (i.e., where a candidate resides, what areas they can cover, and how that assistance will affect current coverage plans in motion). For example, with Naples being a more rural area, a sign-on bonus is more likely to occur, so long as there is not already assistance in this specific area.

Job Summary:


Our client is seeking highly motivated and compassionate Home Health PT Physical Therapists fulltime for our [insert cities/county] Team. As part of our Physical Therapy Team, you will be part of a dynamic Clinical Team who ensures that our patients return and maintain their optimum level of physical functioning.

Job Responsibilities:


Provide physical therapy services to patient as prescribed by a physician in compliance with agency policies and procedures and federal and state regulations.

-Perform initial and ongoing assessments, including OASIS assessments at appropriate time points, to determine level of functioning and patient's ability to perform activities of daily living.

-Develop and revise plans of care, in consultation with the physician and other care team members, which will enable patient to achieve realistic goals of mobility.

-Reports to the physician and designated agency representative patient's reaction to treatment and any changes in the patient's condition.

Required Skills:


Experience: One to two (1-2) years of experience.


Preferred Skills & Experience:


-Minimum of one (1) year of experience in home health care.


Education:


Required Education:


-DPT from CAPTE accredited program.


Preferred Education: -N/A

Required Certifications Licensure:


-Licensure as a physical therapist in the state of Florida.


-BLS from the American Red Cross or American Heart Association.
