Job Description
LPN - Long-Term Care

Schedule
Shift: 3:00 PM - 11:30 PM, 5 days a week, weekends and holidays required.

Assignment Details
Contract Length: 14 weeks
Guaranteed Hours: 40 hours per week

Requirements
- NY LPN Licensure
- BLS Certification (AHA or RQI only)
- 1 year of experience as an LPN in a long-term care facility

Roles & Responsibilities
- Provide direct and indirect skilled nursing care for assigned residents.
- Assess and communicate data regarding residents' physical and psychosocial conditions to the Registered Nurse and the Interdisciplinary Team.
- Collaborate with residents, families, and healthcare teams to develop and revise care plans that reflect residents' needs and promote environmental safety and health.
- Maintain current knowledge and competency in nursing practices and participate in the mentoring or orientation of new staff.
- Lead in professional practice under the direction of a Registered Nurse, considering safety, cost, and impact in care delivery.
