Physical Therapist – Skilled Nursing Facility | Menlo Park, CA

We are partnering with a well-established 160-bed post-acute and skilled nursing facility in Menlo Park, CA to find an experienced, compassionate, and professional Physical Therapist (PT) to join their growing rehabilitation team. This is a permanent, full-time opportunity with an ASAP start — ideal for a clinician who thrives in a collaborative SNF environment.

Position Details:

• Setting: Skilled Nursing Facility / Post-Acute Rehab (160-bed)
• Patient Population: Geriatric
• Schedule: Full-time | Permanent
• Hours: Monday – Friday, standard business hours
• Location: Menlo Park, CA
• Pay Rate: $65/hour
• Start Date: ASAP

About the Role:

As a Physical Therapist, you will evaluate and treat a geriatric patient population recovering from surgery, illness, or injury in a post-acute setting. You will manage an average caseload of 10 patients per day, with a productivity expectation of 87–90%. The facility uses PCC and Net Health as their EMR systems.

You will be part of a team of 5 therapists in a flexible, cooperative environment where collaboration and patient-centered care are at the heart of everything they do.

Key Responsibilities:

• Evaluate and develop individualized physical therapy treatment plans for geriatric patients
• Deliver skilled therapeutic interventions including therapeutic exercise, functional mobility training, and manual techniques
• Maintain 87–90% productivity while delivering high-quality patient care
• Accurately document patient progress and care plans in PCC and Net Health
• Collaborate with an interdisciplinary team including OTs, SLPs, nursing, and physicians
• Supervise Physical Therapy Assistants (PTAs) as applicable
• Participate in discharge planning and care coordination

Requirements:

• Current California PT license (or eligibility to obtain)
• Graduate of an accredited Physical Therapy program (DPT preferred)
• Minimum 1–2 years of SNF or post-acute experience required
• Proficiency or willingness to learn PCC and Net Health EMR
• Strong compliance, professionalism, and communication skills
• Team-oriented, flexible, and patient-focused work style
