Job Title: Registered Nurse (RN) Case Manager – Home Health | Build Lasting Patient Relationships

Company: Home Health Care, Inc.

Territory: Bloomington & Eden Prairie + approximately 35 miles from your home for on-call and weekend coverage

Schedule: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM | 1 Weekend Per Month | 1 On-Call Shift Per Week

Compensation: $85,000 – $130,000 annually plus $6,000 in first-year bonuses

What You'll Do

• Manage a home health caseload throughout Bloomington and Eden Prairie , with on-call and weekend visits within approximately 35 miles of your home.
  
• Complete OASIS assessments and individualized plans of care.
  
• Deliver skilled nursing care and patient education.
  
• Coordinate care with physicians and interdisciplinary team members.
  
• Help patients achieve their goals while remaining safely at home.
  
• Maintain timely and accurate documentation.
  

What We're Looking For

• Active Minnesota Registered Nurse (RN) license.
  
• 3+ years of RN experience (home health experience preferred).
  
• Strong clinical assessment and critical thinking skills.
  
• Excellent communication and organizational abilities.
  
• Reliable transportation for local travel.
