Case Manager

Job Summary : Under direct supervision of the Program Manager, or their designee, the Case Manager will provide support to victims and their children, consisting of emotional and logistical support needed to overcome the challenges of domestic/intimate partner violence, sexual assault, and stalking. Address the potential co-occurrence of chronic homelessness, substance abuse, physical disabilities, and mental illness with an emphasis on providing intensive, trauma-informed case management. Provide direct, domestic violence-specific support services to Housing First Participants' and serve as the liaison with La Casa's project partner.

Essential Functions and Responsibilities :

• Through effective case management, provide comprehensive intakes, safety planning, and assessments that will result in individual service plans;
• Receiving referrals from La Casa's case managers for clients who present an immediate need for housing;
• Working collaboratively with victim advocates to ensure a sensitive response to victims and their children who may be in crisis and/or seeking our support;
• Conducting a brief questionnaire or assessment designed to determine the need for Housing First Services
• Collaborating with on-site consultants/clinicians to provide comprehensive services to high-risk clients;
• Empowering survivors by coordinating linkages and working collaboratively with community-based organizations to ensure accessibility and greater options for clients and their families;
• Working in tandem with the partner agency to support clients and facilitate a monthly check-in meeting to review caseload;
• Maintaining client files for safekeeping, in compliance with grant and/or contract guidelines;
• Complying with program data collection and reporting in accordance with grant requirements;
• Strictly maintaining client files and confidential client communications in compliance with grant and contract guidelines.
• Execution, maintenance, and submission of program documentation and data in accordance with grant requirements, including evaluations and surveys.
• Participating in regularly scheduled staff and case management meetings.
• Participating in weekly case conference/staff meetings with the housing site's administrative team;
• Other duties as assigned.

Minimum Requirements :

• BA/BS in Behavioral Sciences and a minimum of two (2) years verifiable case management experience in the relevant field or an Associate degree with 3 years verifiable case management experience in the relevant field, or a GED/High School diploma with 5 years of verifiable case management experience in the relevant field.
• Significant experience in domestic violence, mental health, substance abuse, homelessness, and physical illnesses.
• Experience working in a dynamic, diverse, multicultural, multilingual setting.
• Knowledge of case management, counseling techniques, peer counseling models, crisis intervention, social milieu, and group facilitation.
• Working knowledge of community resources in the San Francisco area.
• Strong written and verbal communication skills.
• Ability to work as a team member with a strong commitment to collaborative work.
• Ability to approach work with a positive, supportive, and sensitive attitude and demeanor.
• Ability to respond sensitively to women, teens, and their children who may be in crisis and/or seeking support.
• Must be able to fulfill physical requirements of the job (lifting, pushing, pulling, carrying, walking up and down stairs multiple times per day, etc.).
• Understanding of confidentiality and privilege laws governing domestic violence work.
• Verifiable completion of state-mandated 40-hour domestic violence counselor training. Or will be completed during the next scheduled agency 40-hour domestic violence counselor training, including working one overnight shift at La Casa's emergency shelter.
• Clearance through DOJ Live Scan fingerprinting is required. La Casa de las Madres will consider applicants, including those with criminal histories, in a manner consistent with San Francisco's Fair Chance Ordinance.
• Valid CA driver's license may be required (depending on the position), clean driving record, and insurable under agency policy.
• TB (Tuberculosis) clearance
• Bilingual preferred: English/Spanish

Monday - Friday 8:30 a.m. to 5 p.m.
Must be available for evenings, nights, and weekend coverage.
Non-exempt, Full Time (40 hours per week)
