Pediatrics Nurse Practitioner

We are seeking an experienced Pediatric Advanced Practice Provider to join an innovative Advanced Care at Home program in Jacksonville, Florida. This is a unique opportunity for an NP or PA with a strong pediatric acute or critical care background who is interested in expanding beyond a traditional inpatient role while continuing to care for medically complex pediatric patients.

In this position, you will work closely with physicians and a multidisciplinary pediatric care team to support patients as they transition from the hospital to home. You will round on inpatient units, identify and enroll appropriate patients, participate in discharge planning and patient/family education, and continue providing clinical oversight through telehealth once patients return home.

This role offers a rewarding blend of inpatient pediatric care, care coordination, virtual medicine, and program development.
