Ambulatory Case Manager (RN) - Maternal Special Care

Job Description RN Case Manager at Moms2B serves as the Nurse Educator and lead clinical educational specialist, developing and teaching educational programs for the maternal and infant health curriculum in the Department of Obstetrics and Gynecology. The role includes curriculum development, data research, and program evaluation, while also engaging in fundraising and donor awareness activities. Develop and teach educational programs for maternal and infant health. Serve as lead clinical educational specialist for prenatal and postnatal maternal care and childbirth. Provide education and counseling sessions for program participants at various locations. Develop, establish, and oversee educational curriculum for healthy pregnancies tailored to specific patient populations. Plan, develop, and implement personalized education programs based on needs assessments of participants. Review and evaluate the healthy pregnancy program curriculum. Keep abreast of new or revised trends in prenatal and postnatal care. Participate in data research activities and provide summaries of training sessions conducted. Track metrics on maternal and neonatal mortality rates. Identify and obtain needed clinical equipment and supplies. Participate in fundraising and donor awareness activities as necessary. Advocate for patients by ensuring access to appropriate care and services and promoting self‑care management and prevention. Identify and remove barriers to care, coordinating patient services using the nursing process. Collaborate with multidisciplinary teams to ensure comprehensive care plans following evidence‑based standards. Educate patients and caregivers in understanding and managing diagnoses, treatments, and symptoms. Apply care management principles and utilize system and community resources tailored to individual needs. Support NP provider practice. Manage a sub‑population of patients to maximize efficient use of health‑care resources. Minimum Education Bachelor's level degree with a major in nursing required. Minimum Qualifications Bachelor's degree in Nursing required; Master’s degree desired. Current Ohio licensure and current Basic Cardiac Life Support (BCLS) certification required. Minimum of 2 years of experience as an RN preferred; experience in case management desired. Final candidates are subject to successful completion of a background check. A drug screen or physical may be required during the post‑offer process.
