DescriptionJob SummaryDyad partner with the Neuroscience Institute Neuro Genetics' Program Medical Director MD and APRN, in developing, expanding, improving, and sustaining the program across the continuum at a recognized local, regional, and national Center of Excellence (COE) status. Responsible for the development, implementation, review and revision of Program standards, policies, procedures, and protocols. Builds Program structure to meet all selected accreditation and certification body(s) standards, national registry elements, and U.S. News and World Report standards to achieve top ranking. Supports the Programs clinical and treatment database, investigators activities, and multidisciplinary clinics. Delivers patient navigation across the continuum and lifespan.Job Specific DutiesServes as an active member of the Neuroscience Institute Leadership Team.Develops financial, growth, satisfaction, and clinical metrics to reflect performance of the Program. Directs and participates in special projects that promote the growth, efficiency, and operations of the program. Collaborates with the multi-disciplinary team to develop operational and strategic goals and objectives consistent with Angelman Foundation, IRSF, Dup 15 Alliance, and other relevant certifying bodies.Works in a dyad leadership role with the Neuro Genetics' Medical Director to coordinate the development, implementation, review, and revision of Neuro Genetics Program standards, policies, procedures, and protocols based on current clinical practice guidelines. Co-leads the Neuro Genetics Leadership and operations meetings including a) agenda & minutes, b) reports for clinical metrics/core measures, c) actions, timeline, and responsible person; and post multi-disciplinary clinic meeting for patient care planning plus FU to plan of care for each patient. Prepares patient for Multi Disciplinary Clinic ensuring approprianess of setting and availability of testing results. Performs follow up care coordination including concierges appointment setting with Coordinator, complex authorizations, and school forms for all post clinic needs. Follows all Program patients for inpatient admissions and navigate return to outpatient care.Contributes to and supports execution of the Program and Institute's Strategic Plan. Maintains a clinical dashboard including outcomes metrics in the clinical, functional/QOL, and perception of care domains. Identify trends and patterns and opportunities for improvement, maintaining two QI initiatives ongoing. Supportive to the financial dashboard and improvements against targets. Reviews all neuro genetic related order sets annually/on-going with Medical Directors, Nurse Navigator, and Neuro Genetic Program committee.Monitors compliance with neuro genetic metrics and scorecard. Monitors compliance with national epilepsy regulations and guidelines. Identify and participates in performance improvement initiatives.Partners with Clinical Education to identify and address educational needs for staff around topics like target metrics and new guidelines/treatment.Supportive and engaged with COE presentations, publications, and talks. Accountable for U.S. News and World Report Neurology metric reporting and outcomes to achieve top ranking.QualificationsMinimum Job RequirementsMaster's Degree in Nursing. Genetics, Genetic Counseling, or health related fieldLess than 2 years of clinical experience with the neuroscience population, genetics, or program developmentKnowledge, Skills, and AbilitiesLean certification and experience preferred.Proficient in Microsoft Office (Word, Excel, and PowerPoint).Ability to apply mathematical techniques, logical, and analytical thinking to interpret operational/financial data, prepare capital/operating budgets, assess performance improvement, and solve a broad range of problems.Ability to interpret, adapts to, and reacts calmly under stressful conditions.Ability to relate cooperatively and constructively with patients, families, peers, physicians, and other Health System personnel.Knowledge of purchase through payment process.
