Key Responsibilities\n

• Conduct thorough psychosocial assessments and develop individual care plans tailored to the unique circumstances and goals of each patient.

• Provide continuous emotional support and counseling to patients and their families, helping them navigate the challenges associated with terminal illness.

• Facilitate family meetings to discuss care options, coping strategies, and available resources to enhance the patient experience.

• Implement evidence-based intervention strategies that include mindfulness techniques, grief counseling, and crisis intervention to improve mental health outcomes.

• Collaborate with medical teams to ensure an integrated approach to care, addressing both psychological and physical needs.

Patient Population / Client Focus\n

In this role, you will mainly serve patients facing terminal illnesses and their families, providing the support necessary to navigate this challenging phase of life. Your expertise in emotional and psychological support will help clients process their experiences, manage feelings of grief and loss, and find pathways to peace and acceptance.

Qualifications\n

• A Bachelor’s Degree in Social Work is required; a Master’s Degree is preferred.

• Must hold a valid Licensed Clinical Social Worker (LCSW) license in the state of New Jersey.

• Demonstrated ability to carry out comprehensive client assessments and develop effective treatment strategies.

• Excellent communication skills and a demonstrated capacity to work collaboratively within a team setting.

Preferred Experience\n

• At least 2 years of relevant experience in social work, preferably in hospice, palliative care, or a related field.
