RN Field Case Manager - Hospice (South Miami-Dade County) Amedisys Hospice Care This position is based in Hialeah, FL and requires daily travel throughout South Miami-Dade County, beginning at Flagler Rd and traveling South from there. Schedule: Monday-Friday, 8:00 AM - 5:00 PM with participation in an on-call rotation. Requirements Must be fully bilingual in English and Spanish, with the ability to communicate effectively with patients, families, and team members in both languages. On average our orientation for an RN is 7-9 weeks, however is individualized according to your needs and experience. Responsibilities Performs patient assessments and collaborates with the care team to develop and implement a plan of care. Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so. Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies. Records pain/symptom management changes/outcomes as appropriate. Teaches patients, families, and caregivers appropriate patient care techniques. Regularly communicates with clinical manager and care team. Performs on-call responsibilities and on-call services to patients/families as assigned. Other duties as assigned. Qualifications Required: Full bilingual proficiency in English and Spanish Current RN license in the state(s) where you will work Minimum 1 year of strong clinical RN experience in an acute healthcare setting Current CPR certification Preferred: Hospice or home health experience Strong communication and interpersonal skills Ability to work independently and manage multiple priorities Comfortable with daily travel between facilities Proficient in electronic medical record (EMR) systems This position requires a background screening through the Florida Care Provider Background Screening Clearinghouse.
