Qualifications

• 1 years of experience (education or training in a healthcare setting)
• Strong clinical background in a long-term care setting
• Excellent communication, organizational, and problem-solving skills
• Ability to work collaboratively as part of a healthcare team
• Familiarity with MDS processes and regulations (training provided for the right candidate)

Responsibilities

• As a Regional MDS Coordinator, you will be responsible for coordinating the Minimum Data Set (MDS) process across multiple facilities within our region
• This includes ensuring accurate and timely completion of MDS assessments, coordinating with healthcare teams to address quality of care concerns, and maintaining compliance with all relevant regulations and standards
• Your strong clinical background and excellent communication skills will enable you to effectively collaborate with staff, physicians, and other healthcare professionals to achieve our goals
• Coordinate MDS assessments and updates, ensuring accuracy and compliance with regulations
• Collaborate with healthcare teams to identify and address quality of care concerns
• Develop and implement strategies to improve clinical outcomes and quality of care
• Maintain accurate and up-to-date records of MDS assessments and quality care initiatives
• Provide education and training to staff on MDS processes and clinical care standards
• Participate in quality improvement initiatives and contribute to the development of quality improvement projects
• Develop and Maintain Case Mix Index (CMI) for your region
• Work closely with finance team to ensure maximal reimbursement
