Key Responsibilities

• Conduct thorough evaluations for patients in both inpatient and outpatient environments and curate customized treatment plans
• Provide physical therapy treatments centering on enhancing mobility, strength, balance, and overall function
• Collaborate closely with a team comprising nurses, doctors, case managers, and additional rehabilitation experts to maximize patient outcomes
• Guide patients and their families through therapeutic goals, discharge processes, and home exercise education
• Meticulously document patient evaluations, treatments, and progress as per facility standards and regulations
• Engage in interdisciplinary meetings and care team rounds to ensure cohesive and coordinated patient care

Qualifications

• Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution

Certifications

• Valid state licensure as a Physical Therapist
• CPR certification preferred but not mandatory
• Additional certifications in specialized areas (neurological, geriatric, etc.) are advantageous yet not required

Required Skills

• Prior experience in physical therapy roles is highly preferred
• Adept in treating patients with complex medical and physical challenges
