Work Environment

In our rehabilitation facility located in Keansburg, you will immerse yourself in a collaborative, patient-focused environment. You will engage with a diverse patient population, including those recovering from surgeries, injuries, or chronic conditions, and collaborate closely with multi-disciplinary teams.

Key Responsibilities

• Conduct thorough patient evaluations by reviewing medical histories and performing specialized assessments that help form a comprehensive understanding of each patient's needs.
• Communicate evaluation findings clearly to patients and their families, actively involving them in the goal-setting process, which encourages greater patient engagement in their care.
• Implement personalized treatment protocols informed by thorough evaluations, adapting techniques to align with each patient's specific presentation and progress.
• Maintain open lines of communication with other healthcare professionals to consult on patient care as required, documenting significant changes in patients' conditions to revise treatment plans appropriately.
• Handle discharge planning effectively, ensuring that appropriate follow-up care is arranged. Create detailed documentation that traces the patient's rehabilitation journey, includes focused evaluations, and illustrates progress through daily encounter notes and interim summaries.
• Manage time efficiently while meeting productivity expectations, maintaining an organized schedule to optimize patient care.
• Participate in ongoing marketing initiatives for both hospital and community engagement, promoting the services we provide to enhance care access.
• Contribute to departmental goals by participating in regular team meetings and training sessions, and support continuous growth through ongoing education and professional development.
• Foster positive customer interactions by being adaptable and responsive to the unique needs of our patients.
• Engage in initiatives aimed at removing barriers to care, understanding and applying access-related metrics to improve service delivery.

Patient Population / Client Focus

The Physical Therapist will work directly with a variety of patients from diverse backgrounds, including those recovering from orthopedic surgeries, sports injuries, neurological conditions, and chronic pain syndromes. Our personalized approach ensures that each patient receives the tailored care they require to achieve their rehabilitation goals.

Qualifications

• A Bachelor's degree in Physical Therapy
• Current and valid state license for physical therapy in New Jersey
• Basic Life Support (BLS) certification from the American Heart Association is mandatory
• Strong communication and interpersonal skills to effectively interact with patients and families
• Ability to work collaboratively within a multidisciplinary team environment

Preferred Experience

• A minimum of two years of experience in a rehabilitation setting is preferred but not mandatory.
• Prior involvement in chronic disease management or post-operative rehabilitation may be advantageous.
