21st Century Home Health Services (21HHS) is seeking a Part-time Registered Nurse Case Manager for the Novato area. You will assess patients for home care needs, perform SOC admissions and follow-up visits, and coordinate care with physicians and the clinical team. Schedule options include weekend days (Sat/Sun) with possible additional weekdays; autonomy and flexible planning are offered, along with mileage reimbursement, an iPad, and comprehensive Benefits. #J-18808-Ljbffr 21st Century Home Health Services Inc.
