Work Environment\n

This position is entirely remote, allowing the Licensed Clinical Social Worker the flexibility to deliver patient-centered care from a comfortable and personal environment. Our Hospice Center's philosophy emphasizes a multidisciplinary approach to care, which means you'll collaborate with psychiatrists, nurses, and other healthcare professionals to provide a seamless care experience. You will work Monday to Friday, from 9:00 AM to 5:00 PM, offering consistent support to our patients.

Key Responsibilities\n

• Provide high-quality psychotherapy to patients suffering from mental health conditions, including but not limited to anxiety, depression, post-traumatic stress disorder (PTSD), and other emotional disturbances.

• Conduct comprehensive assessments to evaluate patients' mental health status, taking into account their emotional, behavioral, and social functioning.
\n
• Develop and implement personalized, evidence-based treatment plans that address the unique needs and backgrounds of each patient, ensuring cultural competence throughout the process.

• Monitor and document patient progress, making necessary adjustments to treatment plans based on patient feedback and team collaboration.
\n
• Educate and support patients and their families about mental health conditions, treatment options, and community resources, empowering them to make informed decisions regarding their care.

• Maintain the highest ethical standards by ensuring confidentiality and securing informed consent from patients.

Patient Population / Client Focus\n

The LCSW will primarily work with patients at our Hospice Center, providing mental health support to those at critical points in their lives. This may include patients who are coping with terminal illnesses, grief, and end-of-life transitions. Compassion and sensitivity are essential, as you will play a pivotal role in helping patients and their families navigate complex emotional challenges.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current state licensure as a Licensed Clinical Social Worker (LCSW).

• Minimum of 2 years of direct experience providing psychotherapy and mental health services.

• Strong clinical skills with a track record of conducting thorough assessments and developing effective treatment strategies.

Compensation and Benefits\n
