Responsibilities
- Complete initial evaluations and ongoing assessments to determine PT goals and equipment needs.
- Develop and implement individualized rehabilitation plans focused on mobility, balance, strength, gait training, and activities of daily living.
- Deliver direct patient care, oversee progress, document outcomes, and adjust plans to maximize function and safety.
- Educate patients, families, and care teams on treatment plans, safety, and durable medical equipment recommendations.
- Collaborate with the interdisciplinary team to coordinate care, discharge planning, and transition of residents to appropriate levels of assistance.
- Ensure accurate charting and compliance with regulatory and facility standards; participate in rounds and care planning meetings.

Requirements
- Active physical therapy license in New Mexico (or eligibility for NM licensure)
- Doctor of Physical Therapy (DPT) degree from an accredited program
- Experience in skilled nursing facility or geriatric rehabilitation preferred
- Strong evaluation, treatment planning, and documentation skills
- Ability to work a full 8.5-hour day shift and meet guaranteed weekly hours
- Effective communication and teamwork within an interdisciplinary setting
