Key Responsibilities\n

As a Licensed Professional Clinical Counselor, your primary responsibilities will include:

• Assisting primary care providers in identifying, treating, and managing mental health and psychosocial issues, thereby enhancing the overall patient care experience.

• Conducting comprehensive patient intakes that focus on diagnostic and functional evaluations, and subsequently making informed recommendations to the primary care team concerning treatment goals and personalized care plans.

• Delivering brief, focused interventions to patients requiring mental health support, ensuring that they receive timely and efficient care.

• Collaborating with primary care providers to determine which patients would benefit from in-house mental health services and which may need referrals to specialty facilities.

• Following up with patients to monitor their progress, assessing the need for adjustments in treatment approaches as necessary.
\n
• Developing and implementing relapse prevention plans, aiding patients in maintaining stable functioning and resilience.
\n
• Identifying “at risk” patients and devising preventative strategies to address potential declines in their mental health.
\n
• Coordinating the delivery of health services related to behavioral care, facilitating connections with other treatment providers while respecting patient confidentiality.

• Documenting patient progress meticulously in treatment charts, keeping primary care providers informed of any changes in patient status or needs.
\n
• Educating patients on self-management skills and coordinating strategies to help them comply with any medical treatments initiated by their primary care providers.

• Providing consultation to clinic management and interdisciplinary team members regarding behavioral health services, contributing to ongoing program evaluations.

• Upholding the Code of Ethical Conduct for Clinical Professionals in all interactions.

Qualifications\n

The ideal candidate will possess the following qualifications:

• A Master's Degree in Clinical Counseling or a related field.

• A current and valid Licensed Professional Clinical Counselor (LPCC) license.

Compensation and Benefits\n

The salary range for this position is competitive, ranging from USD 87,000 to 105,000 annually, commensurate with experience.
