Key Responsibilities\n

The LPCC will play a vital role in our integrated care model by:

• Collaborating with primary care providers to identify and address mental health issues among patients, ensuring comprehensive treatment plans.

• Conducting thorough patient intakes, which involve diagnostic assessments and functional evaluations, utilizing evidence-based practices to recommend tailored treatment goals.

• Providing brief, focused therapeutic interventions for clients experiencing mental health challenges, utilizing techniques such as cognitive behavioral therapy (CBT) and solution-focused therapy.

• Monitoring patient progress through regular follow-ups and consultations, adapting treatment strategies as necessary based on observed outcomes and patient feedback.

• Developing detailed relapse prevention plans that empower patients to maintain stability and promote ongoing personal development and recovery.

• Documenting patient assessments, treatment plans, and progress notes in compliance with agency protocols and Federally Qualified Health Center (FQHC) guidelines.

• Educating patients on self-management strategies and providing them with resources to improve their mental health and lifestyle choices.

Qualifications\n

The ideal candidate should possess:

• A minimum of a Master’s Degree in Clinical Counseling.

• Current licensure as a Licensed Professional Clinical Counselor (LPCC).
