Key Responsibilities\n

• Conduct ongoing assessments to evaluate the needs of your clients, tailoring your approach to foster their unique therapeutic journey.

• Actively participate in communication among supervisors, direct care staff, families, and other stakeholders regarding clients’ progress, ensuring a cohesive care plan.

• Develop and provide a tentative schedule for client appointments and home visits to monitor and support your caseload effectively.

• Coordinate home visits as necessary to conduct observations, engage in chart reviews, interview staff, and perform data analysis pertinent to client care.

• Ensure that the implementation of program plans is executed thoroughly and monitored for quality and effectiveness.

• Engage in interdisciplinary team meetings, contributing insights and collaborating on client care strategies.

• Complete monthly and quarterly progress notes meticulously to track client development and treatment outcomes.

• Organize training sessions for support staff to ensure everyone is proficient in program implementation and documentation practices.
