Case Management Worker (LSW or LCSW) - Inpatient

Overview Provides social work services to patients and families within assigned areas in order to meet identified psychosocial needs, as well as to enhance problem‑solving and coping capacities and to facilitate timely discharge by arranging post‑discharge services. The social worker assists with new Senior Nursing Facility (SNF) placement, addresses acute social determinants and works collaboratively with all other members of the healthcare team, ensuring clear communication and care planning to all patients. Qualifications Licensed Social Worker (LSW) – Illinois Department of Financial and Professional Regulation (IDFPR) Licensed Clinical Social Worker (LCSW) – Illinois Department of Financial and Professional Regulation (IDFPR) Education: Bachelor’s Degree in Social Work; Master’s Degree in Social Work Work Experience: (required – experience in community or hospital social work setting) Responsibilities Investigate cases admitted with “no insurance” or “self‑pay” to determine eligibility for financial assistance. Evaluate patients for abuse/neglect and make appropriate referrals. Involve patient and family in discharge planning decisions. Keep director/manager informed of problematic cases, especially those involving legal or risk management issues. Assist and advocate for patients in obtaining financial resources and government entitlements. Provide social work intervention to patients and families. Offer support and planning in cases of death, adoption, and fetal demise. Respond to referrals from healthcare team members to identify available facilities and services for case‑specific needs during and post‑hospitalization. Help identify Health‑Care Surrogate as required by the Health‑Care Surrogacy Act. Assess uninsured or under‑insured patients for Illinois Medicaid eligibility and/or Carle Financial Assistance Program eligibility; provide direction with application(s). Complete PASRR assessments to facilitate discharge to extended‑care facilities. Complete mandated reports of suspected abuse/neglect of minors, disabled adults, and seniors; cooperate with investigative bodies. Find and, if possible, arrange transportation for patients to and from medical appointments and/or home from the Emergency Department/Hospital. Educate patients on and connect them with community resources (e.g., non‑skilled home help, meals, housing resources, charity assistance programs, support groups). Provide empathic listening and emotional support to patients in person or over the phone. Compensation and Benefits The compensation range for this position is $30.07 per hour to $51.72 per hour.
