Key Responsibilities\n- \n
- Facilitate engaging individual psychotherapy sessions using evidence-based practices to support clients’ emotional and mental health needs. \n
- Conduct group therapy sessions that promote peer support and healing among patients and their families. \n
- Align your services with our organization's policies, adhering to Mental Health and Substance Abuse Standards while ensuring compliance with your practice scope. \n
- Perform comprehensive assessments of patient and caregiver needs to formulate an appropriate and effective care plan. \n
- Evaluate clients' functioning across work, family, and daily routines, identifying areas requiring further support, resources, and intervention. \n
- Collaborate continually with patients and caregivers to modify care plans based on ongoing evaluations of their needs. \n
- Develop therapeutic care plans with established goals, collaborating closely with physicians and other healthcare providers within the hospice team. \n
- Provide crisis counseling and therapeutic interventions to individuals, groups, and families, ensuring prompt and effective support during critical moments. \n
- Set specific, measurable goals in therapy and consistently track progress towards those goals, adjusting methods and approaches as necessary. \n
- Demonstrate sensitivity to the diverse and comprehensive needs of patients and families as they navigate the hospice experience. \n
You will primarily work with individuals and families dealing with terminal illnesses, providing not only psychological support but also emotional and social resources critical in hospice care. This unique clientele requires a nuanced understanding of the mental health challenges associated with end-of-life scenarios, where you will support patients in finding meaning, peace, and acceptance during their remaining time.

- A Master’s Degree in Social Work from a program accredited by the Council on Social Work Education. \n
- Current and valid Social Work License in accordance with state regulations (LMHC, LMFT, LICSW). \n
