Work Environment\n

In this role, you will work from the comfort of your home, connecting with clients through telehealth platforms and remote communication tools. Our community health clinic model allows for flexible scheduling, typically Monday through Friday from 8:00 AM to 4:30 PM, while occasional evening or weekend hours may be needed to address client requirements. Your flexibility in scheduling will ensure we provide uninterrupted care to our clients. Additionally, you may be required to be on call to assist in urgent situations.

Key Responsibilities\n

• Perform comprehensive assessments of clients’ psychosocial needs, identifying emotional, mental, and social factors impacting their overall well-being.

• Collaborate closely with clients and their families to create personalized care plans that reflect specific goals, preferences, and desired outcomes.

• Provide counseling and emotional support, helping clients navigate challenges and transitions in their lives, such as health crises or changes in living situations.

• Advocate for clients, ensuring they have access to community resources, services, and support networks that can aid their recovery and well-being.

• Work collaboratively within interdisciplinary teams, including nurses and healthcare professionals, to enhance overall care coordination.

• Conduct regular check-in visits with clients to monitor their emotional well-being, assess their living conditions, and address any emerging issues or safety concerns.

• Facilitate communication among family members involved in client care and resolve any conflicts that may arise.

• Educate clients and their families on available community resources and support groups, promoting social interaction and improving quality of life.

• Maintain detailed and accurate documentation of assessments, care plans, and interactions in compliance with regulatory standards.

• Stay informed on the latest social work practices, ethical guidelines, and regulatory policies to ensure compliance in all interactions.

Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.

• A valid and current state licensure as a Licensed Clinical Social Worker (LCSW) in California.
