Key Responsibilities\n

• Conduct detailed psychosocial assessments by utilizing interviews, observations, and thorough reviews of medical records to discern emotional, social, and environmental factors that are pivotal to each patient’s recovery journey.

• Provide empathetic counseling and therapeutic support to patients and their families, effectively addressing common adjustment difficulties and fostering resilience and emotional stability during times of change.

• Collaborate closely with our team of healthcare professionals—including physical therapists, occupational therapists, and speech-language pathologists—to craft individualized treatment plans that effectively address each patient's unique psychosocial needs and ensure holistic care.

• Assist patients in navigating and accessing vital community resources such as local support groups, vocational rehabilitation services, or housing assistance programs, thereby promoting their overall recovery and wellness.

• Maintain comprehensive and accurate documentation of patient interactions and treatment outcomes within the electronic health record (EHR) system, ensuring compliance with all relevant regulatory requirements.

Qualifications\n

Compensation and Benefits\n
