Registered Nurse, Home Health – Corpus Christie, TX Job Summary This full‑time position provides skilled nursing care to patients in their homes within the Corpus Christie area. The RN will travel during business hours (Monday – Friday, 8 am to 6 pm) and will be scheduled for on‑call and weekend coverage on a rotating basis. Additional overtime and differential pay are available. Position Highlights & Responsibilities Perform nursing assessments and develop, implement, and evaluate plans of care in collaboration with physicians, patients, and caregivers. Review OASIS data and re‑evaluate patient needs at appropriate time points. Provide skilled nursing services, including medication administration, wound care, therapeutic treatments, and education. Plan and implement patient health teaching and counseling to promote self‑care and independence. Participate in health promotion and disease prevention activities. Initiate appropriate preventive and rehabilitative procedures following physician orders. Coordinate services and scheduling with the Clinical Team Manager and Clinical Team Coordinator; recommend additional home health services within 24 hours of Start of Care. Assess patient condition during each visit and communicate findings to the Clinical Team Manager daily. Perform care‑management duties: notify physicians of changes, obtain orders, reassess for recertification, supervise LVNs and home health aides, attend case conferences, and coordinate discharge planning. Document all communications with patients, families, physicians, and other disciplines. Prepare written plans for Home Health Aides as needed. Communicate verbally and in writing in a clear, positive, and cooperative manner. Required Qualifications Graduate of an accredited nursing program. Current unrestricted Texas RN license or Smith‑Kett‑Lee (compact) license. BLS certification, or ability to obtain within 30 days of employment. At least 1 year of home‑health or related experience. Experience with wound care. Proficiency in Microsoft Office (Outlook, Word, Excel). Driver’s license and reliable transportation for patient visits within the designated area. Preferred Qualifications OASIS experience. Physical Requirements Ability to lift and transfer patients. Pay & Benefits The hourly pay range for this role is $28.94 to $51.63, depending on local labor markets, education, experience, and certifications.
