Outpatient RN Case Manager - Admissions & Care Coordination

Harbor Healthcare System

Harbor Healthcare System is seeking an experienced RN Case Manager for an outpatient role in Missouri. You will be an integral member of the care team, responsible for admissions, assessments, discharges, and coordinating appropriate care for each patient and family. The role requires MO RN licensure, strong communication, and the ability to travel.
