Field-Based Community Health Advocate
IMPaCT Care
IMPaCT Care in Baltimore is seeking Community Health Workers to serve in partnership with Kaiser Permanente. You will work directly in clients' homes and community settings, building trust and coordinating care to improve health outcomes while navigating social determinants of health. Ideal candidates have a high school diploma, 1–3 years in community outreach or related work, and a valid driver’s license.
