Key Responsibilities\n

• Conduct comprehensive assessments to identify and address the specific mental health needs of clients during initial consultations and ongoing sessions.

• Actively participate in case management and team meetings to discuss client progress, treatment plans, and effective strategies to improve patient outcomes.

• Coordinate home visits and outreach as needed for individuals requiring in-depth assessments and progress monitoring.

• Utilize electronic medical records (EMR) software to document treatment plans, progress notes, and communications in compliance with legal and ethical standards.

• Engage in collaborative treatment planning, ensuring that program plans are effectively implemented and adjusted based on client feedback and observable outcomes.

• Facilitate training sessions for support staff on program implementation and documentation, ensuring all members are equipped to provide consistent care.

• Maintain accurate and confidential records, serving as a liaison with families and community resources to support individual treatment goals.
