RESIDENT CARE COORDINATOR

Resident Care CoordinatorThe Resident Care Coordinator (RCC) is responsible for overseeing and coordinating the care of residents in an assisted living community. This role ensures that residents receive high-quality, individualized care while maintaining compliance with state regulations and company policies. The RCC works closely with caregivers, families, healthcare providers, and administrative staff to support residents' health, safety, and overall well-being.Key ResponsibilitiesMonitor residents' health status and update care plans as neededCoordinate care services, including medication management and wellness supportSupervise and support caregiving staff to ensure quality care deliveryCommunicate regularly with residents, families, and healthcare providersEnsure compliance with state regulations and community policiesAssist with resident admissions, transfers, and dischargesMaintain accurate and up-to-date resident records and documentationRespond to emergencies and ensure proper incident reportingParticipate in care conferences and interdisciplinary team meetingsQualificationsExperience in assisted living, senior care, or memory careStrong knowledge of resident care planning and regulatory complianceExcellent communication and interpersonal skillsLeadership and team management experienceAbility to multitask and respond effectively in a fast-paced environmentCPR/First Aid certification (or willingness to obtain)Skills & CompetenciesCompassion and empathy for residentsStrong organizational and problem-solving skillsAttention to detail and accuracy in documentationAbility to lead and motivate a teamConflict resolution and decision-making skillsWork EnvironmentAssisted living community settingMay require evenings, weekends, or on-call availabilityPhysically active role (standing, walking, occasional lifting)RequirementsArizona Certified Caregiver License (Required)Valid Level One Fingerprint Clearance CardCurrent CPR/First Aid certificationMust show proof of freedom from TB before client contactProvide and maintain current Food Handlers CertificationClean driving record and valid driver's license (preferred for transport duties)Memory Care CertificateClear the Department of Economic Security, Department of Aging and Adult Services Central Registry Background CheckAbility to lift 50 lbs and assist with mobility as neededStrong work ethic, reliability, and compassion for individuals with special needs Everlasting Services
