Position Overview

This role involves coordinating healthcare services to optimize patient outcomes through assessment, evaluation, collaboration, and management of care delivery systems. The focus is on implementing cost-effective strategies that ensure high-quality care within the evolving healthcare landscape for the designated patient population.

Key Responsibilities

• Maintain a safe and clean work environment, adhering to unit safety and infection control protocols.
• Coordinate services to maximize patient outcomes effectively.
• Assess, evaluate, and collaborate with healthcare teams and participate in service line activities.
• Implement cost-efficient care strategies aligned with current healthcare standards.
• Manage utilization review, discharge planning, and other healthcare service systems.
• Serve as a liaison and facilitator among patients, families, healthcare providers, and insurance personnel.
• Oversee direct admission processes and bed assignments for patients admitted through physician offices or transfers from other facilities (emergency department focus).
• Provide coverage for holiday shifts as required.

Qualifications

• Minimum of three years clinical and case management experience in an acute care environment preferred.
• Familiarity with Prospective Payment System, JCAHO standards, and regulatory requirements related to utilization management and discharge planning is desirable.
• Strong written and verbal communication skills.

Education

• Bachelor's degree in nursing or a related health field preferred.

Licenses

• Current Registered Nurse license in California required.
