Sr Clinical Lead, Chronic Disease Management & Preventive Care

Office Location: We strongly prefer candidates who are able to work out of our New York City office location (505 Eighth Avenue, New York, NY 10018). We have a hybrid schedule of working 2 days in the office and 3 days remote. We are also open to candidates based elsewhere in the United States.

GENERAL STATEMENT OF RESPONSIBILITIES:

CAI is seeking a Senior Clinical Lead to provide vision and leadership for our National Technical Assistance Program’s work to provide high-impact technical assistance that supports Federally Qualified Health Centers and Look-Alikes, collectively referred to as health centers, to deliver comprehensive, high-quality primary health care, improve chronic disease management, nutrition, and preventive services, improve operational effectiveness, efficiency, and quality, and address emergent public health needs and priorities. This individual will lead the design and advancement of CAI's clinical care improvement strategy nationally, drawing on direct, hands-on experience leading improvement initiatives - including standardization of evidence-based clinical recommendations in practice including use of team-based care models, population health management, and partnerships that connect patients to the non-clinical and social supports that drive better outcomes — in a health center setting. This includes translating that experience into models, tools, and guidance that other health centers can adopt. The Senior Clinical Lead will also inform the development of technical assistance (TA) materials and tools and deliver group- and individual-level TA.

MAJOR TASKS AND RESPONSIBILITIES (may include, but are not limited to):

• Serve as CAI's senior clinical leader and national subject-matter expert on chronic disease management, preventive screening, and care coordination, setting and advancing the vision, strategy, and priorities for this body of work across the Technical Assistance Program.
• Lead efforts to design, deliver and continuously improve technical assistance to strengthen access to, quality of, and outcomes in primary care, drawing on personal, direct experience leading successful improvement initiatives — with demonstrated outcome improvements — in a health center setting.
• Define and advance evidence-based and promising-practice models for chronic disease management, maternal health, child and adolescent, and preventive care (e.g., team-based care models, population health management and registry-based outreach, closed-loop referrals) appropriate for primary care and health center settings.
• Lead strategies to strengthen care continuity and follow-up, and to build partnerships that connect patients to the clinical and non-clinical services and supports improved patient outcomes.
• Provide clinical guidance to CAI leadership and project teams on strategy, quality standards, and emerging issues.
• Lead the development of TA materials and tools that operationalize clinical interventions, best practices and improvement activities (e.g., implementation playbooks, curricula, workflow guidance, eLearning modules, factsheets, infographics), working with content, evaluation, communications, and materials development staff.
• Facilitate group- and one-on-one TA to health center staff and leadership as a mechanism for translating strategy and tools into practice, and identify, prepare, and support other CAI staff and consultants to deliver TA as needed.
• Partner with evaluation and project staff to define outcome measures, evaluate the effectiveness of chronic disease and preventive care TA and tools, and drive continuous improvement based on findings.
• Build and maintain positive working relationships with health center staff and leadership, partner agencies, funders, and other stakeholders.
• Represent CAI's work in internal meetings and in meetings with funders and external partners, as required.
• Report on strategy, TA planning, implementation progress, and key findings to project teams, partner agencies, and other stakeholders at regular intervals.
• Support funder reporting activities and efforts to disseminate and share project findings related to chronic disease management and preventive care.
• Ensure all chronic disease and preventive care strategy, TA, and resources adhere to CAI's and funders' quality standards.
• Other duties as necessary/assigned.

MINIMU M EDUCATION, EXPERIENCE AND OTHER SKILLS REQUIRED:

• Registered Nurse (RN), Nurse Practitioner (NP), Physician Assistant (PA), or Physician (MD/DO), with a Bachelor's, Master's, or doctoral degree in Nursing, Medicine, or a related clinical field required.
• 10+ years of experience in clinical practice, including direct leadership of improvement initiatives within a primary care or health center setting, with a demonstrated track record of improved patient and program outcomes.
• Experience working at or with health centers as part of interdisciplinary care teams, applying evidence-based interventions and team-based care models (e.g., population health management, registry-based outreach, closed-loop referrals) in primary care settings.
• Applied knowledge of improvement frameworks, implementation science, and the design of clinical strategy to drive practice change and improved outcomes.
• Excellent analytic, strategic thinking, and presentation skills, including strong interpersonal, oral, and written communication skills.
• Excellent planning skills, highly organized, and able to manage multiple concurrent priorities.
• Ability to establish rapport and work productively and sensitively with diverse stakeholders, community members, and partners.
• Active RN, NP, PA, or physician licensure preferred.

COMPENSATION:

The salary range for this position is $130,000 - $165,000, depending on experience.
