Work Environment\n

This position offers flexibility as the primary work setting is remote, allowing you to manage your schedule effectively. You will coordinate with clients via virtual meetings, phone calls, and occasional onsite visits. While work hours are primarily Monday through Friday, from 8:00 AM to 4:30 PM, occasional evening and weekend hours may be required to ensure that all client needs are met. The ability to adapt to the evolving demands of your caseload will be essential.

Your core responsibilities will include:

• Conducting comprehensive assessments of seniors' psychosocial needs, carefully analyzing emotional, mental, and social factors that influence their well-being.

• Collaborating closely with clients and their families to develop tailored care plans that cater to specific goals, preferences, and challenges.

• Providing counseling and emotional support to seniors and their families to help them navigate transitions such as relocation, loss, or health changes.

• Advocating for clients' rights and facilitating access to vital community resources and services they require.

• Working seamlessly with interdisciplinary teams, including nurses, healthcare professionals, and caregivers, to deliver cohesive and comprehensive care.

• Performing regular virtual or in-person visits to monitor clients' conditions, assess living environments, and identify any safety concerns.

• Facilitating communication among family members involved in the care of senior clients to resolve conflicts and ensure the best outcomes.

• Educating clients about community resources, support groups, and programs aimed at enhancing social engagement.

• Maintaining detailed and accurate documentation of client interactions, assessments, care plans, and progress notes in compliance with regulatory standards.

• Staying updated on current practices and ethical standards in social work to ensure high-quality service delivery.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Current and valid LMSW licensure in the state of Colorado.

Preferred Experience\n

While the required experience includes two years in social work, candidates with a background in community health or geriatric care will be preferred. Familiarity with local community resources, social services policies, and client advocacy will enhance your ability to succeed in this role.

Compensation and Benefits\n
