About the Position

Job Title: Care Manager Profession: Care Manager Specialty: ED Duration: Full-time Shift: Day Hours per Shift: 9 AM - 5 PM Experience: Minimum of five years of clinical experience as a Registered Nurse. License: RN License as a Registered Nurse in the State of New York required. Certifications: Baccalaureate degree required. PRI and Screen certification within three months of employment is required. Master’s Degree and Bilingual Spanish Certification in Case Management preferred. Must-Have: Experience with care coordination, utilization review, and discharge planning. Computer literacy required. Description: The primary goal of the Care Manager is to optimize member health care and delivery of care experience. This role is expected to lead to cost savings through improved quality of care. Care Managers engage with members to understand their needs, environment, providers, support system, and available services. They are responsible for assessing and evaluating member needs and acting as creative, efficient, and resourceful problem solvers. In collaboration with the member’s care team, a plan of care with individualized goals and interventions is developed, implemented, and outcomes evaluated. Care Managers address members' problems and needs, including clinical, psychosocial, financial, and environmental factors. They provide services to members of varying ages, risk levels, clinical scenarios, cultures, financial means, social support, and motivation. Engagement in a collaborative relationship is key, empowering members to self-manage their physical, psychosocial, and environmental health for lifelong well-being. They prepare member-oriented plans of care with members, caregivers, and health care providers while integrating cultural sensitivity and privacy practices. Participation in interdisciplinary rounds is expected. Care Managers ensure that plans of care have individualized goals and interventions and communicate these plans to the Primary Care Physician. They address gaps in care and members' social determinants of health issues, linking them to available resources. Care management support is provided during transitions of care, ensuring member and caregiver understanding, especially concerning language barriers or cognitive limitations. Training is offered for relevant chronic diseases, preventive care, medication management, home safety, etc. Complex care management is included, ensuring access to care, reducing unnecessary hospitalizations, and appropriately referring to community supports. Advocacy for members involves assisting them in addressing challenges and making informed choices regarding their clinical status and treatment options. Employing critical thinking and judgment is essential when dealing with unplanned issues. Maintaining knowledge of chronic conditions and using data to track and trend outcomes and clinical information is required.
