Location: Yonkers, New York
Salary: $60,000 annually
Schedule: Will begin Mon-Fri 11am to 8pm, program will grow adding another case manager, schedule will then change to 3 days 9am to 9pm plus one half day.
Position Summary
The Case Manager provides comprehensive case management and supportive services to individuals participating in a transitional housing program and drop-in center serving adult men in Yonkers, New York . The Case Manager works directly with residents and drop-in participants to assess needs, develop individualized service plans, connect individuals with appropriate community resources, and support progress toward housing stability, employment, benefits, health and wellness, and greater self-sufficiency.
This position requires a compassionate, organized, and resourceful professional who can effectively engage individuals experiencing homelessness or housing instability. The Case Manager should have a strong understanding of housing and employment resources, public benefits, and social-service programs available throughout Westchester County and be comfortable developing relationships with community-based organizations and government agencies.
Knowledge of HMIS (Homeless Management Information System) is highly desirable. Bilingual English/Spanish skills are also highly desirable.
Essential Duties and Responsibilities
- Conduct comprehensive intake interviews, assessments, and ongoing evaluations of residents and drop-in center participants to identify housing, employment, financial, health, behavioral health, benefits, and other service needs.
- Develop and maintain Individualized Service Plans (ISPs) in collaboration with participants, establishing realistic goals and measurable steps toward housing stability and self-sufficiency.
- Maintain regular contact with assigned participants to monitor progress, address barriers, and update service plans as needed.
- Provide housing-focused case management, including assistance identifying appropriate permanent and supportive housing opportunities.
- Assist participants with housing applications, documentation, identification, benefits verification, and other requirements necessary to secure permanent housing.
- Develop knowledge of and relationships with Westchester County housing resources, landlords, housing providers, supportive housing programs, and community-based organizations .
- Assist participants in accessing employment and workforce-development opportunities, including job-readiness programs, vocational training, educational opportunities, resume assistance, and employment referrals.
- Connect participants with appropriate Westchester County and local resources , including public benefits, health and behavioral health services, substance-use treatment, legal services, food assistance, transportation, identification/documentation services, and other supports.
- Develop and maintain strong working relationships with community organizations, government agencies, nonprofit providers, housing organizations, workforce-development programs, and other referral partners .
- Advocate on behalf of participants when necessary to help remove barriers to housing, employment, benefits, and other essential services.
- Provide crisis intervention and problem-solving support while maintaining appropriate professional boundaries.
- Maintain accurate, timely, and confidential case notes, assessments, service plans, referrals, progress reports, and other required documentation.
- Enter and maintain participant information in HMIS and/or other required case-management databases in accordance with program and funding requirements.
- Track participant outcomes, including housing placements, employment, benefits, referrals, and other program goals.
- Participate in case conferences, staff meetings, interdisciplinary team meetings, and coordination meetings with outside providers.
- Work collaboratively with Program Management, Security, Administration, Maintenance, and other staff to support a safe, respectful, and service-oriented environment.
- Maintain participant confidentiality and comply with all applicable program policies, privacy requirements, and professional standards.
- Complete incident reports and other required documentation accurately and within established timeframes.
- Perform additional duties as assigned by the Program Director or senior management.
Community Linkage & Resource Development
A key component of this position is building a strong network of resources that can assist participants in moving toward stability and permanent housing. The Case Manager will actively identify and maintain connections with organizations providing:
- Permanent, supportive, and affordable housing
- Employment and workforce-development services
- Vocational training and educational programs
- Public benefits and entitlements
- Mental health and substance-use services
- Primary and specialty healthcare
- Legal and immigration-related referrals, when appropriate
- Food and nutrition assistance
- Clothing and emergency assistance
- Transportation resources
- Financial literacy and budgeting assistance
- Identification and documentation assistance
- Veterans' services and other specialized programs
- Additional Westchester County and community-based resources
Qualifications
- Bachelor's degree in Social Work, Human Services, Psychology, Sociology, Counseling, or a related field preferred. Relevant case-management and human-services experience may be considered.
- Previous experience working in homeless services, transitional housing, shelters, supportive housing, drop-in centers, or community-based social services strongly preferred.
- Knowledge of Westchester County housing, employment, public benefits, and social-service resources strongly preferred.
- Experience developing community partnerships and making effective referrals to outside organizations.
- Knowledge of HMIS or similar case-management/data systems is helpful.
- Bilingual English/Spanish is highly desirable.
- Strong documentation, organization, and time-management skills.
- Ability to manage a caseload and prioritize multiple participant needs and deadlines.
- Strong interpersonal and communication skills and the ability to engage individuals from diverse backgrounds respectfully and professionally.
- Ability to work collaboratively as part of a multidisciplinary team.
- Strong crisis-management, problem-solving, and conflict-resolution skills.
- Proficiency with Microsoft Office, email, and electronic case-management systems.
- Ability to maintain confidentiality and professional boundaries at all times.

Work Schedule
This is a full-time afternoon/evening position , with the anticipated schedule beginning at approximately 12:00 PM or 1:00 PM and ending at approximately 8:00 PM or 9:00 PM . The later schedule is designed to ensure case-management services are available during periods when residents and drop-in center participants are more likely to be present and in need of assistance.
Compensation
Salary: $60,000 annually , plus applicable benefits in accordance with organizational policy.
