Position Overview

The Jewish Community Health Center, located in the scenic region of Nassau, New York, is actively seeking a dedicated and skilled Nurse Practitioner (NP) to join our team on a full-time basis. This role will be pivotal in enhancing our outpatient services offered through the Center for Advanced Cardiac Care (CACC). The ideal candidate will play a key role in patient management, working closely under the guidance of the Chief Nurse Practitioner and the Medical Director in our specialized cardiology division.

Work Environment

As a Nurse Practitioner at our health center, you will operate in an onsite setting dedicated exclusively to home care services. In this environment, you will be engaged in direct patient interactions, assessments, and treatments, collaborating with proficient cardiologists and a multidisciplinary team of healthcare professionals. Your typical work week will run from Monday to Friday, 9 AM to 5 PM, with no required nights, weekends, or on-call duties, creating an excellent work-life balance.

Key Responsibilities

• Work closely with a team of cardiologists to develop and implement patient care plans tailored to individual needs.
• Manage chronic conditions such as hypertension and hyperlipidemia by conducting regular assessments and adjusting treatment plans as necessary.
• Provide outpatient-focused Electrophysiology services to patients presenting with abnormal ECG results, including monitoring and managing implanted cardiac devices.
• Assess and manage acute cardiovascular conditions including coronary artery disease, heart failure, and peripheral artery disease, ensuring timely interventions and referrals when needed.
• Conduct stress tests and analyze results to guide treatment decisions effectively.
• Document comprehensive patient histories and perform thorough physical examinations; prescribe medications in accordance with clinical guidelines.
• Facilitate timely referrals to specialized cardiology colleagues and participate in case discussions to enhance inter-team communication.
• Educate patients and their families on health management strategies, promoting self-care and wellness initiatives.

Patient Population / Client Focus

The Nurse Practitioner will serve a broad spectrum of patients, primarily focusing on individuals experiencing cardiovascular health issues. This role involves direct care for patients ranging from young adults to the elderly, each presenting distinct clinical challenges. You'll need to approach patient care with cultural competence and sensitivity, ensuring that each individual feels comfortable and supported throughout their healthcare journey.

Qualifications

• A Bachelor’s Degree in Nursing is required; a Master's or Doctoral degree in nursing is preferred.
• Completion of an accredited Nurse Practitioner program with appropriate certification is mandatory.
• A current and valid Nurse Practitioner license to practice in New York is essential.
• Certification in Advanced Cardiovascular Life Support (ACLS) is a plus.

Preferred Experience

While we welcome applications from both experienced Nurse Practitioners and new graduates, candidates with previous experience in cardiology—whether in an inpatient or outpatient context—will be highly valued. This role requires a good understanding of cardiovascular diseases, treatment protocols, and patient education.

Compensation and Benefits

This full-time position offers a competitive annual salary ranging from $120,000 to $160,000, commensurate with experience and qualifications. In addition, our benefits package includes:
