Position Overview

Gotham Enterprises Ltd is actively seeking a skilled and compassionate Nurse Practitioner (NP) to join our dynamic outpatient practice at the Center for Advanced Cardiac Care (CACC) located in Albany, New York. This full-time, onsite position offers a unique opportunity to provide high-quality care through concierge medicine and house calls. You will work under the leadership of the Chief Nurse Practitioner and collaborate with the CACC Medical Director as well as a dedicated team of healthcare professionals.

Work Environment

The CACC operates in a supportive and advanced medical facility focused on outpatient services, integrating concierge medicine and house call approaches. Our facility is equipped with advanced technologies, and our practice emphasizes teamwork among Nurse Practitioners, Physicians, Registered Nurses, and other clinical staff. This role involves direct patient engagement in a flexible setting that supports work-life balance while requiring a hands-on approach to care.

Key Responsibilities

• Conduct comprehensive patient assessments including obtaining detailed histories and performing thorough physical examinations.
• Order and interpret necessary diagnostic tests, developing accurate diagnoses and appropriate management plans for both acute and chronic conditions.
• Independently deliver care to a designated group of patients while collaborating with multidisciplinary teams to meet individual needs.
• Review and integrate internal and external medical records and reports to inform treatment plans.
• Create and implement strategic, goal-directed health plans based on established protocols, incorporating anticipatory guidance and education.
• Facilitate patient referrals for hospital admissions when needed and coordinate comprehensive discharge plans with inpatient teams.
• Perform timely follow-ups, modifying treatment plans in collaboration with supervising physicians as required.
• Provide education and counselling to patients and their caregivers on disease process management and self-care strategies.
• Engage with NP peers and administrative staff to efficiently triage daily patient inquiries and maintain effective communication with referring providers.
• Utilize electronic medical records (EMR) for precise documentation of patient interactions and treatment progress.
• Participate in research initiatives, contributing to the continuous improvement of clinical practices through incorporation of evidence-based findings.
• Adhere to safety protocols and contribute to the maintenance of a supportive, therapeutic environment for both patients and staff.
• Undertake additional responsibilities as assigned by leadership to enhance operational efficiency and patient care delivery.

Patient Population / Client Focus

This position primarily focuses on adult patients dealing with complex cardiac conditions, including advanced heart failure, pulmonary arterial hypertension, and those requiring post-transplant care. The collaborative nature of our practice allows for personalized treatment plans tailored to meet each patient’s unique needs, enhancing their overall health and quality of life.

Qualifications

• Master's degree in Nursing as a Nurse Practitioner, with specialized training in adult care.
• Current licensure as a Registered Nurse and Nurse Practitioner in New York State.
• Certification as a Nurse Practitioner (NP) by an accredited board.
• Current certifications in Basic Life Support (BCLS), Advanced Cardiac Life Support (ACLS), and possession of DEA and NPI credentials.
• Proficient with electronic medical record systems and Microsoft Office Suite, including Word, PowerPoint, and Excel.

Preferred Experience

A minimum of two years’ experience in a clinical setting, preferably within cardiology, is highly desirable. Additionally, familiarity with managing patients undergoing advanced therapies, such as intravenous or inhaled medication support and mechanical circulatory support, will be advantageous in this role.

Compensation and Benefits

The position offers a competitive salary range of USD 120,000 to USD 220,000 per year, depending on qualifications and experience.
