Full-Time Nurse Practitioner for Comprehensive Community Health Care

Position Overview

We are seeking a dedicated and passionate Nurse Practitioner to join our thriving team at One Medical Health Care Clinic, located in Albany, New York. This full-time position offers the opportunity to make a meaningful impact on the health of families and individuals within a school environment. You will be responsible for delivering holistic health care, ensuring accessible quality services while working within a school setting, primarily from Monday to Friday, 9 AM to 5 PM.

Key Responsibilities

• Conduct thorough health assessments and screenings tailored to the unique needs of individuals and families within the community context.
• Diagnose and develop treatment plans for acute and chronic medical conditions, emphasizing preventive care and health promotion strategies.
• Lead and implement health education programs in collaboration with community organizations, focusing on key health issues relevant to the school population.
• Provide culturally sensitive health care while ensuring advocacy for patient rights and their access to necessary healthcare services.
• Manage referrals and coordinate follow-up care to maintain continuity in health services for patients, effectively navigating insurance and community resources.
• Maintain complete and accurate patient medical records to comply with the regulations and organizational standards.
• Engage in community outreach initiatives and health fairs, providing health screenings and vital health information to families.
• Stay informed on community health trends, evidence-based practices, and relevant public health policies to enhance care delivery.

Patient Population / Client Focus

This role will primarily focus on serving a diverse patient population within a school setting, including children, adolescents, and their families. You will cater to a wide range of health needs, from preventive check-ups and chronic condition management to acute health issues—ensuring inclusive care for all backgrounds and health statuses.

Qualifications

• A Master's or Doctoral degree in Nursing from an accredited institution.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) issued by recognized professional bodies, such as the ANCC or AANP.
• Possession of a current, unrestricted Nurse Practitioner (APRN) license in New York State, including valid DEA and NPI numbers.
• Solid clinical assessment and diagnostic skills, with proficiency in employing evidence-based practices in patient care.
• Exceptional communication and interpersonal skills to foster a trusting relationship with diverse populations.

Preferred Experience

• A minimum of 2 to 5 years of clinical experience, ideally in primary care or community health settings.
• Hands-on experience with electronic health records (EHR) systems to streamline patient care documentation and management.
• Demonstrated ability to work autonomously as well as collaboratively within a multidisciplinary team.

Compensation and Benefits

• A competitive annual salary ranging from $150,000 to $160,000, commensurate with experience and qualifications.
