Work Environment

This position is based in Albany, New York, where you will work onsite within patients' homes. Key Responsibilities

• Conduct thorough and comprehensive patient assessments through detailed physical examinations, comprehensive medical histories, and necessary diagnostic tests.
• Collaborate closely with cardiologists and healthcare teams to create, implement, and adjust individualized treatment plans, ensuring optimal patient outcomes.
• Administer medications as prescribed, monitor patient reactions and progress, and modify treatment plans based on clinical evaluations.
• Educate patients and their families on essential aspects of cardiovascular health, including preventive measures, dietary recommendations, and adherence to medication regimens.
• Provide emotional support and counseling to patients and their families, aiding in navigating the challenges associated with heart disease.
• Document all patient interactions meticulously using our electronic medical record (EMR) system to ensure accurate patient history and continuity of care.
• Participate in case reviews and clinical meetings to stay informed about the latest advancements in cardiology and enhance the quality of care provided.

Patient Population / Client Focus

The primary patient population for this role includes individuals diagnosed with various cardiovascular diseases such as hypertension, heart failure, and coronary artery disease. Your work will involve managing acute and chronic conditions, providing preventive care, and focusing on health promotion. You'll be dedicated to addressing the specific needs of each patient, ensuring they receive compassionate and competent care in their home environment.

Qualifications

• Master's degree in Nursing from an accredited institution, focusing on advanced clinical practice.
• Current and valid licensure as a Nurse Practitioner in the state of New York.
• Proven clinical expertise in cardiology or related fields, with strong analytical and diagnostic skills.
• Excellent communication skills, enabling effective interactions with patients, families, and healthcare team members.

Preferred Experience

• A minimum of two years of experience as a Nurse Practitioner in cardiology or a related specialty.
