Key Responsibilities

• Provide comprehensive health assessments for patients in their homes, identifying health needs and creating customized care plans.
• Diagnose and manage a variety of health issues, delivering treatments in accordance with established clinical guidelines and protocols.
• Utilize electronic health records (EHR) systems to document patient encounters and care provided, ensuring compliance with regulatory requirements.
• Educate patients and their families about preventive health measures and support them in making informed healthcare decisions.
• Collaborate with a team of physicians, nurses, and specialists to coordinate patient care effectively and efficiently.

Patient Population / Client Focus

Qualifications

• A Bachelor’s Degree in Nursing from an accredited institution.
• Current certification as a Nurse Practitioner, demonstrating clinical competency and commitment to ongoing learning.
• An active and unrestricted nursing license in New York State.
• Three years of clinical experience, preferably in community health, public health, or home care settings.
• Strong interpersonal and communication skills, empowering you to work effectively with diverse patient populations.

Preferred Experience

Compensation and Benefits

The Nurse Practitioner position offers a competitive annual salary ranging from USD 150,000 to 160,000, commensurate with your experience and educational qualifications. In addition to a competitive salary, we provide a comprehensive benefits package that includes:
